Attendance log: Not required by the state
Portfolio: Not required by the state
K.S.A. 72-3120 requires instruction for a period 'substantially equivalent' to the public school term in your district. It sets no number itself; the public-school benchmark it points at is K.S.A. 72-3115, which is 186 days OR 1,116 hours for grades 1-11 (465 hours for kindergarten, 181 days / 1,086 hours for grade 12). Days and hours are alternatives, not both, and your own district's calendar may run longer.
Register the non-accredited private school's name and address with the State Board of Education (K.S.A. 72-4346, free online form), teach through a competent instructor, and run a term substantially equivalent to your district's. No approval, curriculum review, attendance log, portfolio, testing or reporting is required.
Whatever Kansas asks for, a daily log covers it: the date, which student, minutes or hours of instruction, the subjects touched, and a line of notes. Add a photo of one piece of work a week and you also have a portfolio. Keep the log all year and the end-of-year form is a matter of copying totals.

Do I have to keep attendance records to homeschool in Kansas?
Kansas does not require an attendance log by statute. Most families keep one regardless: it is the fastest way to answer a district question, it feeds the year-end paperwork, and it settles any later dispute about days of instruction. A log with the date, hours and subjects for each school day is enough.
